The Dental Van is coming to Carver Middle School soon (the date will be announced at a later time). Forms were sent home with students and can be turned in to Nurse Winter. Nurse Winter has extra forms, if needed.

***You must bring your old Chromebook, charger and 2 signed forms. These forms were emailed to students and are also available below. They were also included with the report cards. Report cards are still available at the front office for pick up between the hours of 8 am and 4:30 pm.
